100K Lives and Beyond

Promoting the Culture of Safety

The 100K Lives Campaign identifies six specific and proven hospital practices that dramatically improve patient care and save lives. It is rooted in landmark research by the Institute for Healthcare Improvement indicating that as many as 98,000 Americans may die from preventable deaths in hospitals each year due to medical mistakes.

We helped bring this national initiative to hospitals throughout our service area, providing $450,500 to fund "100K Lives and Beyond" in collaboration with the Delmarva Foundation.

In 2005, the 100K Lives and Beyond Collaborative extended its proven lifesaving practices to intensive care units and operating rooms, to further develop a "culture of safety" at more area hospitals.

The culture of safety: best practices – learned, then shared.

A "culture of safety" within the hospital setting is created and reinforced by structured learning sessions and best practices collaboration between institutions:

  • Prevention of adverse drug events by careful administration of medications based on dosage reconciliations
  • Prevention of central line infection by use of interdependent, scientifically grounded steps called "the Central Line Bundle"
  • Prevention of surgical site infections by reliable and timely delivery of appropriate peri-operative antibiotics and other related procedures
  • Prevention of ventilator-associated pneumonia by following a series of interdependent and scientifically grounded procedures called "the Ventilator Bundle"

A total of 34 hospitals throughout the mid-Atlantic region currently test and implement strategies for improving handoffs and transitions, with a particular emphasis on medication reconciliation.
